What are Hammertoes?

Hammertoe impacts the middle toes, causing one or more toes to bend upward at their middle joint, resulting in varying levels of discomfort and a noticeable hammertoe deformity.

Once stiffened into position, a hammertoe can rub against shoes and cause painful corns and calluses, leading to hammertoe pain. Some individuals with hammertoe have difficulty walking, shoe inserts, and participating in everyday activities, seeking hammertoe to relieve pain.

Lifestyle changes can sometimes help manage this condition, but certain individuals will opt for hammertoe surgery, also known as toe-straightening surgery, to alleviate the pain and correct the hammertoe deformity.

How Do We Treat Hammertoes?

Hammertoe Surgery

Depending on the severity of the condition, either conservative hammertoe treatment options and lifestyle readjustments or surgical treatment are necessary to treat hammertoe and its causes.

Surgical hammertoe procedures can include joint resection or bone fusion to straighten the affected toe or toe bends upward, effectively providing hammertoe correction. Hammertoe surgery is performed on an outpatient basis, so the patient can return home the same day as their surgery, beginning their hammertoe surgery recovery process.

Conservative Hammertoe Treatment Options

Before choosing surgery, a number of conservative treatment options are available for those suffering from hammertoe, including the following.

Wear Different Shoes:

wear shoes

A larger, softer shoe can relieve the direct pressure or pressure from adjacent toes. Make sure that the shoe has enough room at the end (about the width of your thumb) and that the toe box (the front of the shoe) is high enough to allow your toes to move freely.

Treat Corns and Calluses:

If a corn or callus has formed, reduction by either professional sharp debridement (shaving) or consistent filing will give temporary relief. Use caution with over-the-counter corn removal medication or pads. This type of medication is a type of acid that can cause an ulceration or infection if not used properly or in excess.

Focus on Arch Support:

Temporary arch supports or custom molded orthotics can give support and minimize the gripping or clawing effect of toes from a flattening archway. These devices are a good solution for long-term treatment with or without surgical intervention.

Surgical Treatment for Hammertoe

To remodel and straighten the toe joint, a minimal (0.5 to 1 centimeter) incision is made at the top of the toe. The surgeon will then remove a small portion of the joint to fully extend and straighten the toe. To ensure that the toe will remain straight, a small pin is placed in the middle of the toe. Both absorbable and metallic options are available and do not need to be removed.

Another surgical option involves bone fusion. The surgeon goes through the above process but also fuses the two bones that create the impacted toe joint. A wire, pins, or screws are inserted to help them fuse naturally and then removed after the toe heals.

  • Pin & stretch technique for problems with toe muscles, tendon transfer, ligament, and fascia
  • Specialist with the foot in a surgical procedure

If indicated by the patient in either case, skin lesions and corns can be removed during the procedure. Otherwise, they will naturally fall off three to four or few weeks after surgery.

Surgical hammertoe procedures are performed on an outpatient basis, using a local anesthetic or sedation, and allowing for the patient to return home after the surgery. Patients are advised not to bear complete weight on their foot until 48 to 72 hours of partial weight bearing after the outpatient procedure, marking the beginning of hammer toe surgery recovery.

What is a Hammertoe?

A hammertoe is a deformity that causes a toe to bend or contract at the joint, giving it a hammer-like appearance. This condition typically affects the second to fifth toes and is more common in females.

What Causes Hammertoes?

Hammertoes can be caused by a variety of factors, including tendon and muscle imbalances, flat feet or over-pronation, heredity, trauma, arthritis, and wearing tight or poorly-fitting shoes.

What are the Symptoms of Hammertoes?

Common symptoms of hammertoes include pain and pressure from footwear at the top of the bent toe, corn formation on the top of the joint, redness and swelling at the joint contracture, restricted or painful toe joint motion, and pain in the ball of the foot at the base of the affected toe.

When Should I Seek Care for Hammertoes?

Early intervention is crucial in managing hammertoes effectively. If you notice toe contracture, irritation, or any of the symptoms mentioned above, it's essential to visit a podiatrist. Early conservative treatment can slow the progression of the deformity and potentially prevent the need for surgery.

What are the Treatment Options for Hammertoes?

Treatment options for hammertoes include conservative therapy, which aims to manage the symptoms associated with hammertoes, and surgical intervention, which may be necessary when conservative treatments are unsuccessful or the hammertoe is severe.
